You call your internet provider because your service is down. A chatbot greets you: "Hi! I'm here to help. What can I assist you with today?"

Yes, you've tried restarting your router. You've tried it three times. You type: "Yes, I restarted the router. I need a technician."
The bot: "I understand your frustration! Let's try a few more steps. Can you check if the lights on your router are blinking?"
You're now arguing with a machine while your income evaporates. There's no way to reach a human. The chatbot won't let you through.

The Loop Problem
The most common chatbot failure is the loop. Customer describes a problem. Bot offers a scripted solution. Customer says it didn't work. Bot offers the same solution with different wording. Or offers the next scripted solution. Customer says that didn't work either. Bot runs out of scripts and starts over from the beginning.
The customer is trapped. They can't get past the bot. They can't get a human. They're repeating themselves to a machine that doesn't remember what they just said.
Some companies do this intentionally. They know a certain percentage of customers will give up and hang up. That "deflection" looks great on a dashboard. In reality, it's driving customers to competitors.
The Fake Empathy Problem

Fake empathy is worse than no empathy. When a human agent says "I'm sorry this happened," it carries weight because the agent is a person who chose to say it. When a bot says it, it's a string of text triggered by a keyword. The customer feels patronized.
The Wall Problem

Scripted chatbots (decision trees) follow predefined paths. "If customer says X, respond with Y." They work for very simple, predictable scenarios. They fail completely when the customer's issue doesn't match any script. And customers don't follow scripts.
LLM-based chatbots (GPT, Claude, etc.) generate responses on the fly. They're more flexible than scripted bots. But they hallucinate. They make up policies. They give confidently wrong answers. Both approaches share the same flaw: they try to generate a response to the customer's message. That's the wrong goal.
